INTERESTED IN APPLYING FOR A HEALING HEARTS THROUGH YOGA SM INTERNSHIP
We only accept a small number of students for each module to create a very personal training experience. You should submit your application early. The next program begins in March 31, 2011. Submit in the same package ALL the following items. Application packets that are not complete will be returned:
2. Two Letters of Recommendation, one should be from a yoga instructor you have trained extensively with.
3. Letter of intention that includes a statement on a. your interest in the Internship b. your understanding that the program requires 100 hours of volunteer service and 50 classroom hours to be successfully completed c. what you hope to contribute to the program d. what you hope to learn from the program
4. A copy of your current Driver's License or Passport
5. Proof of Measles, Mumps, Rubella, Varicella (Chicken Pox), Hepatitis B vaccinations and a recent TB test (either doctors records or titers are acceptable) Please note this is a mandatory requirement of our affiliated hospitals. If you are not immunized you can not be accepted into the internship.
6. A copy of your current Yoga Alliance membership card
7. A copy of your declaration page from your current professional liability insurance policy. The policy must include your name.

9. Tuition is $995.00 (please note: there is an additional 2.5% fee for credit or debit card payments). You may pay online by clicking HERE or include a check (do not send cash) with your application and supporting materials. Tuition includes one tee shirt (mandatory social identifier to be worn at hospital during your shifts) and the Healing Hearts through Yoga workbook. Your deposit will be returned to you if you are not accepted into the program. The balance of tuition is due prior to the start of your program.
10. If you would like YA CEU's add $20 to your tuition payment
Cancellation Policy. If you are accepted into the program and you withdraw 30 or more days before the start date for your module you will receive a full refund less a $50 administrative fee. If you withdraw within 30 days of your start date there will be a $100 administrative fee. If you withdraw on or after the start date with or without notice you tuition is forfeited and there will be no refund. NOTE: All expenses are your sole responsibility. Expenses can include but are not limited to immunizations, background checks, professional liability insurance, tuition, travel, social identifier tee shirt, accommodations, and meals.
